This was the end result of removing the bush and bringing in the extra stable dirt.  This raised the area about 8 inches.

It always helps to have the right tools - especially since they had to mix 15 bags!

They brought a pre-built 3 x 5 form built with 2x6s.  He leveled and staked it before pouring the concrete.
